Melasma Mastery: Causes, Treatments, and Prevention
By Dr. Amina Arafat Shima • Aesthetic Consultant
Melasma is one of the most common, yet challenging pigmentary disorders I see in my clinic. Often called the "mask of pregnancy," it requires a patient, multi-modality approach rather than a quick fix.
What Triggers Melasma?
Unlike simple sun spots, Melasma is hormonally driven. The melanocytes (pigment cells) in your skin become hypersensitive to triggers. The main culprits include:
- UV Exposure: Even minimal sun exposure can flare melasma instantly.
- Hormonal Changes: Pregnancy, birth control pills, and thyroid dysfunction.
- Heat: Interestingly, heat from cooking (thermal energy) can also stimulate pigment.
Dr. Shima's Treatment Protocol
Because Melasma is stubborn, I avoid aggressive treatments that cause inflammation (which breeds more pigment). Instead, I use a "Soft & Safe" approach:
1. Medical-Grade Topicals
We start with tyrosinase inhibitors—ingredients like Tranexamic Acid, Kojic Acid, and Azelaic Acid—to stop pigment production at the source.
2. Q-Switched Laser Toning
We use advanced laser technology at low energy settings. This "shatters" the pigment without heating the surrounding tissue, allowing your body to clear it naturally.
3. Oral Therapeutics
In resistant cases, oral medications (prescribed after checking your medical history) can dramatically reduce vascularity and pigmentation.
